What is dchome.ca?
dchome.ca is an application (app) for Canadian Emergency Department physicians and healthcare providers to send condition-specific instruction sheets to patients by text message (SMS message).
The app is designed to support the discharge conversation between physician and patient, not replace it. See the Canadian Medical Protective Association (CMPA) guidance on informed discharge for more on this.

Mission
Our mission is to facilitate high-quality, efficient patient discharge from Emergency Departments in Canada by making it easy to deliver condition-specific instruction sheets directly to patients.
Information sources
The instruction sheets on this site come from various open-access sources, including:
- The Canadian Discharge Instruction Set (CDIS), a nascent project aimed at providing a high-quality collection of public domain instructions doctors can provide to patients who are sent home (discharged) from hospitals in Canada. CDIS instructions are given for a specific condition, general in nature, written at a grade 5 reading level or below, kept concise, and aimed to be relevant to the Canadian context.
- Emergency Care BC, which provides patient information sheets for a wide range of emergency conditions.
Our team
This website was designed and built by Bryan Renne with early design and technical advice from Puneet Kapur. Both are Emergency Medicine Physicians in Saskatoon with backgrounds in Computer Science. Their partner and research co-lead for the CDIS is James Stempien, also an Emergency Medicine Physician in Saskatoon.
